To book an appointment with a physician at the London Travel Clinic to complete the MTO medical form, please complete the following steps:

1) Click here 

2) Click “no”

3) Click “In person or Phone Visit” 

4) Click Ontario Truck Driving School MTO Physical (6th option) 

5) Choose a date and time that works for you. The address is 1695 Wonderland Road North and the price is $130 if paid cash, $150 otherwise.
